Waiver Form for non-members

For you to use our ice in an upcoming event, you must fill out and sign our Participant Agreement Form.

Age Requirement

If you are under 18, you need to submit a hardcopy of the waiver. Please download and print the waiver form, sign it, have a parent sign it, and bring it to the club or mail it.
